Case Management
|

| Intake |
Prospective clients who request or are referred for services are screened and evaluated for eligibility through a brief information gathering and decision making process. Information is gathered on the client and family history, current problem(s) as well as other services the consumer may currently be using. |
| Client Assessment |
Completion of the Client Assessment will provide the case manager with information which will help in fully understanding the client’s current situation, as well as those problems or concerns which may arise in the future. |
| Development of Service Care Plan |
Once the assessment process is complete, specific treatment goals, objectives and outcomes are identified by the client, therapist, and case coordinator. Specific objectives are identified and assigned to the appropriate person and, active participation of the client is encouraged. |
| Implementation of Service Care Plan |
Obtaining and coordinating services which occur through inter-agency referrals or provision of the services directly by appropriate persons within same agency. Provision of services may also include educating the client and providing support to enable the client to access services. |
| Monitoring of Service Care Plan |
An ongoing evaluation of the service goals will enable the client to have continual feedback from the therapist as to the child's progress as well as addressing any obstacles to completing objectives in the Service Care Plan. |
| Update Revision of Service Care Plan |
Service Plans are intended to be flexible and are updated on an as needed basis. |
| Closure |
Discharging the client from the case management process due to client request or agency termination. This may be due to sucessful completion of all services or may occur prematurely. |

Therapeutic Interventions

| Sand Tray |
This is one form of play therapy involving a sand tray and miniatures. The child is allowed to create a world in the sand. |
|
Art |
Through drawings, a child may be able to show the therapist what they are feeling or experiencing when they are unable to talk about what is happening or how they feel. |
| Play |
Play therapy allows the therapist to assess and understand children's play and to use it in assisting the child in coping with difficult emotions and in finding solutions to their problems. By safely confronting their problems in the protected play therapy environment, children can find creative solutions. Play therapy allows children to change the way they think about, feel toward, and resolve their problems. |
